Behavior Modification Plan

In order to assist students with self-awareness and proper social interation the resource team will use the following behavior management system. This system is only effective in the resource room.

Objectives:

The student will demonstrate each of the following behaviors:

Follow all classroom rule
Complete all assignments
Staying on task
Stay in seat

Procedures:

The teacher will establish a set of rules along with appropriate consequences for the student.
The teacher and student will complete a Student/Teacher Contract agreeing on the goal of the student, the consequences for failure to meet the goals, and the rewards if the goals are met.
The teacher will show positive reinforcement whenever the student displays the appropriate behaviors (praise, treats, free time, more computer time etc.)
The teacher will complete a Behavior Rating Sheet reporting the student’s behavior for each day of the week for six weeks.
The teacher will constantly redirect the student in times of off-task behaviors.

Consequences

1^st offense- Verbal Warning

2^nd offense- Name on board & conference with student

3^rd offense- Check by name & contact parent by phone or note home

4^th offense- Discipline report to office

Reward:

Point System:

A. 4 points will be given each day for weekly prize

Each student must earn at least 15 points each week, students earning 18 to 20 will receive a special prize.

Evaluation:

The student’s special education teacher (Ms. Wells) will complete a behavior rating sheet of the student’s behavior each day for six weeks.
